Standardized templates
Following the publication of the third edition of the Environmental Technical Manual (Doc 9501), Volume IV — Procedures for demonstrating compliance with the Carbon Offsetting and Reduction Scheme for International Aviation (CORSIA), revised standardized templates in the format of spreadsheets are available for download below. These templates are available in all six ICAO official languages.
Template of Emissions Monitoring Plan (from aeroplane operator to State)
An aeroplane operator shall submit an Emissions Monitoring Plan to the State to which it is attributed for approval by the State in accordance with Annex 16, Volume IV, Part II, Chapter 2, 2.2.2.
It is recommended that an aeroplane operator uses the updated standardised Emissions Monitoring Plan template available for download below to develop the Emissions Monitoring Plan for submission to its State.
Template of Emissions Report (from aeroplane operator to State)
An aeroplane operator shall submit an annual Emissions Report to the State to which it is attributed in accordance with Annex 16, Volume IV, Part II, Chapter 2, 2.3.1.
It is recommended that an aeroplane operator uses the standardised updated Emissions Report template available for download below to develop the Emissions Report for submission to its State.
Template of CORSIA eligible fuels supplementary information to the Emissions Report (from aeroplane operator to State)
If an aeroplane operator seeks to claim emissions reductions from the use of CORSIA eligible fuels, the aeroplane operator shall provide supplementary information to the Emissions Report using the updated standardised template available for download below.

A new standardized template of a State Order of Magnitude Checklist for Emissions Report in the format of a spreadsheet is available to States for download below.
Template of Emissions Unit Cancellation Report (from aeroplane operator to State)
An aeroplane operator shall submit an Emissions Unit Cancellation Report to the State to which it is attributed should it have total final offsetting requirements for a given three-year compliance period, in accordance with Annex 16, Volume IV, Part II, Chapter 4, 4.3.1.
A new standardized Emissions Unit Cancellation Report template in the format of a spreadsheet is available to States for download below.

An updated standardized Verification Report template in the format of a spreadsheet is available to verification bodies for download below.
